Friedreich ataxia is the most common form of hereditary ataxia and is caused by mutations in the frataxin gene. Although acylcarnitine profiles are altered in several inherited ataxias, their role in Friedreich ataxia is unknown. The objective of the study was to assess whether acylcarnitines are increased in the serum and cerebrospinal fluid (CSF) of patients with Friedreich ataxia. Eleven patients with pathologically confirmed Friedreich ataxia and two patients with sporadic Friedreich ataxia were studied. Also, twenty-two healthy individuals served as controls. Acylcarnitine concentrations in serum and CSF were quantified by liquid chromatography-tandem mass spectrometry. Acylcarnitines levels were significantly increased in both CSF and serum of Friedreich ataxia patients. Acylcarnitine profiles in CSF and serum reveal a higher concentration of large-chain acylcarnitines and significant reduction of C2-C6 acylcarnitines in Friedreich ataxia patients compared with healthy controls. CSF acylcarnitine profiles reflect the absence of ataxia in Friedreich ataxia patients with clinically negative nerve conduction study. Friedreich ataxia is associated with an abnormality of acylcarnitine metabolism, which could be related to mitochondrial dysfunction, leading to an impairment of energy metabolism in this disease.[The role of nephron in diffuse coagulopathy in acute alcoholic intoxication]. Acute coagulopathy, a frequent life-threatening condition in patients with acute alcoholic intoxication, has a complex pathogenesis. It may be associated with disseminated intravascular coagulation (DIC), reduction in coagulation factors and decreased fibrinolysis. The severity of coagulation activation has been correlated with the impairment of renal function. To evaluate the correlation between the degree of renal dysfunction and DIC, renal function and coagulation activation were evaluated in a cohort of 56 alcoholic patients. DIC was observed in 22 (39.3%) patients. On admission, renal function was determined in all patients. There was a significant correlation between 55cdc1ed1c
